Loan Forms
Federal Loan Request Form This is included with your preliminary award letter.  Use this form to request federal subsidized and unsubsidized loans, up to the maximum amount listed on your preliminary award letter.

Forms:  Applicants must submit a Free Application for Federal Student Aid (FAFSA) and a UVA School of Medicine Application to apply for federal and/or school-funded aid.  The UVA Application can be printed out from the link above.  The completed application should then be mailed to the UVA Financial Aid Office.  Signed copies of federal tax returns and W2s are also required to complete an application.

Parental Financial Information:  Applications for school-funded aid require parental financial information along with your own (and your spouse's if applicable) on the FAFSA and medical school form.  Exceptions to this policy are extremely rare.  No exception will be considered without a written request to the financial aid director prior to application.

Extenuating Circumstances:  The application process provides opportunity for explanation of circumstances such as parental or student costs associated with caring for handicapped children, unusual medical expenses, recent death of a parent, etc.  With documentation of special circumstances, our counselors may be able to reduce an expected family contribution or increase the student budget.

Income Tax Forms:  Although the FAFSA accommodates estimated income tax information, we are not able to make an official aid offer before confirming actual income figures from the prior calendar year.  To verify income reported on the FAFSA, parents (of school-funded aid applicants), applicants and spouses who file federal income tax returns will be required to provide signed copies of the prior year's return (including all schedules and forms filed), along with all W2s.

Application Deadlines: Entering students whose applications are complete (all forms) by April 1 will receive preliminary notification of eligibility by May 1.  Applicants whose files are incomplete can expect delayed notification.

Students offered admission after February 1 should make application within 30 days of acceptance. Those still waiting for an offer of admission on March 1 are encouraged to submit applications so that they can be evaluated quickly once they receive an offer.

Returning students should submit their application materials by April 15th to ensure timely processing of their financial aid.
